Seeking individual to provide job coaching services to customers with all types of disabilities who are interested in becoming employed. Duties include: assisting with search for job shadowing experiences, teaching job tasks at the site, working with employers to negotiate tasks and responsibilities, providing and coordinating transportation, coordinate billing and authorization and report writing. Must have high school diploma and experience as a job coach, teacher or employment specialist. Experience working with people with disabilities preferred. E.O.E.
House Coordinator - PT - Portage Co.
Seeking individual to work with people having mental health issues in our residential
program. Duties include providing assistance with daily living skills, medication monitoring, maintaining safety and completing inventory of supplies. Must be able to work various shifts and every other weekend covering both group homes. High school diploma required. Valid Ohio driver’s license and good driving record a must! Must have LSW, CT, PC or SWT and be available to work nights and weekends. Must have a clean driving record and experience with Qualifacts. Prefer PCC or LISW. E.O.E.
Prescreener - PRN - Trumbull Co.
Seeking individual to provide crisis intervention and clinical assessment of persons experiencing a behavioral health crisis at the Access Center or any of the contracted hospitals. Will work with the emergency department staff to determine a disposition and then coordinate hospitalization or return to the community. Individual will take phone calls at the Access Center to provide crisis support, information and referral services. Must have a BA in related field and LSW or LPC. Will be required to work both on-site and in an on-call capacity. Midnight, afternoon, and day shift coverage required. On-call weekend work is required. MA and LSW, LPC, LISW or LPCC preferred. The ideal candidate should be kind, patient, and willing to assist frail senior adults with toileting, transferring, and eating. Must be a STNA and physically able to lift 50 lbs. Six months to one-year experience working with older adults in home health care or nursing home environment is preferred, but not necessary. Must have group experience and willingness to participate in group counseling.
