Case Manager - PRN - Portage County

Seeking individuals to provide intensive, case management services to individuals with severe and persistent mental health issues in Portage County. The Case Manager will assist with accessing community based resources, housing, employment, making appropriate referrals and linking the individual with ongoing mental health services. Must have BA/BS in mental health or related field or 5 years experience in mental health or related field. Must have and maintain excellent driving record. Prior case management experience preferred. E.O.E.

Seeking dependable individual who is looking for PRN work in a crisis residential setting. Responsibilities include working with team, monitoring client safety and coordinating treatment with team. Willing to work different shifts, weekends and rotate holidays. Must have BA in related field or previous experience. Must have and maintain excellent driving record. Emergency Mental Health Therapist - PT - Portage County

Seeking individual to provide emergency mental health evaluations, diagnostic assessments, crisis interventions, and supportive services to both the general population and mentally ill people of all ages. Must have Ohio licensure as LSW or PC; excellent customer service skills; clean driving record; and crisis or mental health experience. Must be able to work evening and weekend hours and rotate holidays (25-32 hours per week). LISW/ PCC preferred. E.O.E.

LPN - PT - Portage County

Seeking a licensed practical nurse for approximately 20 hours per week for a behavioral health provider in Portage County. Duties include: assessments, establishing treatment plans, monitoring progress, coordinating services, and working with clients and families. Must be an Ohio licensed LPN, have a clean driving record, and two years psychiatric experience. E.O.E.

Prescreeners - PT and PRN - Trumbull County

Seeking individuals to provide crisis intervention and clinical assessment of persons experiencing a behavioral health crisis at the Access Center or contracted hospitals. Will work with the emergency department staff to determine a disposition and then coordinate hospitalization or return to the community. Individual will take phone calls at the Access Center to provide crisis support, information and referral services. Must have a BA in related field and LSW or LPC and be available for some on call responsibilities. MA with RN, LPCC or LISW and previous crisis experience preferred. Good driving record a must. Recovery Coach - PT & PRN - Stark County

Seeking dependable individuals to work with people having mental health issues in our residential “housing first” setting. Responsibilities include working with individuals, monitoring safety, and coordinating with community team. Must have BA/BS in mental health or related field or high school diploma and five years experience in the mental health field. Must be willing to work different shifts, weekends, and rotate holidays. Ohio driver’s license, clean driving record, access to a car and willingness to transport is a must. Experience working with individuals with psychiatric disorders preferred. E.O.E. 

STNA - PRN - Adult Day - Portage County

Coleman Adult Day Services is seeking an exceptional STNA to become a member of its caregiving team. The ideal candidate should be kind, patient, and willing to assist frail senior adults with toileting, transferring, and eating. Must be registered with the State of Ohio as a STNA and physically able to lift 50 lbs. Must be available on an “as needed” basis between the hours of 7:30 am and 5:30 pm, Monday–Friday. Six months to one-year experience in a long term care facility preferred, but not necessary. No nights/no weekends. Wage $10.50 hour.  E.O.E.

Training Consultant - Contract - Allen, Auglaize and Hardin Counties

Training Consultant will work with people with all types of disabilities to help them learn tasks associated with employment. Responsibilities include: work with employers and assist with job accommodations, provide documentation of the services provided and assessment of skills related to the job, provide job coaching for Community Based Situational Assessments and employment. The Training Consultant must be able to work a very flexible schedule to include evenings and weekends. Must have BA in related field, one year experience and clean driving record. CRC and vocational evaluation experience preferred. Experience teaching basic computer and Microsoft office skills a plus. E.O.E.
