The Case Manager will assist with accessing community based resources, housing, employment, making appropriate referrals and linking the individual with ongoing mental health services. Must have BA/BS in mental health or related field or 5 years experience in mental health or related field. Must have and maintain excellent driving record. Prior case management experience preferred. House Monitor - Trumbull County
Coleman is seeking a House Monitor to oversee the completion of chores, safety and maintenance issues, order supplies and provide information to clinical staff as needed at our residential facility. Must have a desire to work with transition aged youth, ages 18-26. Must have a high school diploma, basic computer skills, able to reside at the home, and work a flexible schedule. Housing Technician - Allen, Auglaize and Hardin Counties
Seeking individuals to work with residents to help improve hygiene, room cleanliness, cooperation with others, and encourage residents to meet specified goals and objectives. Individuals will assist in completion of admissions, new resident/family orientation, discharges, shift reports, and chart client progress. Must have high school diploma and clean driving record. Prescreeners - Trumbull County
Seeking individuals to provide crisis intervention and clinical assessment of persons experiencing a behavioral health crisis at the Access Center or contracted hospitals. Will work with the emergency department staff to determine a disposition and then coordinate hospitalization or return to the community. Individual will take phone calls at the Access Center to provide crisis support, information and referral services. Must have a BA in related field and LSW or LPC and be available for some on call responsibilities. MA with RN, LPCC or LISW and previous crisis experience preferred. Good driving record a must.
